The person in charge (PIC) was unable to demonstrate in a verifiable way the one other employee on site was informed of the employee health policy on the symptoms/diseases to report to the health department.// Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: The person in charge (PIC) reviewed and signed the employee health agreement policy with the employee.

Observed a food employee prepare food with two sets of gloves on, touching frozen chicken bites then removing the top gloves, and then touching their glasses with the second pair of gloves.// If used, single-use gloves shall be used for only one task such as working with ready-to-eat food or with raw animal food, used for no other purpose, and discarded when damaged or soiled, or when interruptions occur in the operation. (P) Corrective Actions: The employee discarded both sets of gloves and washed their hands.//
