   Employee health reporting

   Facility could not demonstrate that employees are aware of health policy in a verifiable way. Facility has employees sign as a part of on boarding but could not show the module that was viewed. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: Could not correct on-site. Please correct by 10/15/2026.
