    The PIC could not demonstrate in a verifiable manner that any employees have been informed of their responsibility to report certain symptoms and foodborne illnesses. ***SECOND CONSECUTIVE VIOLATION OF THE SAME CODE PROVISION***// Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: The PIC and co-owner had all of the employees sign the employee health agreement forms during the inspection.//

    Observed a sanitizer bucket and food utensils (knife, grill brush) in the handwashing sink by the three compartment sink. **SECOND CONSECUTIVE VIOLATION OF THE SAME CODE PROVISION***// A handwashing sink shall be maintained so that it is accessible at all times for employee use and not used for purposes other than handwashing. An automatic handwashing facility shall be used in accordance with manufacturer's instructions. (Pf) Corrective Actions: The items were removed and the hand sink was sanitized.//
