   Employee health reporting

   Two employees did not have signed health forms or a way to demonstrate that they were aware of health policy in a verifiable way. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: PIC (person in charge) had employees sign forms.

The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer.
