    Observed an employee working that the person in charge could not demonstrate in a verifiable manner that the food employee was informed of his responsibility to report symptoms and/or diseases.// Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: The person in charge had the employee sign a health agreement that describes the symptoms and disease that he is responsible to report.//

    Observed that the ice machine in the back across from the double door freezer had a black mold like substance on the top near where the ice is made.// Equipment food-contact surfaces and utensils shall be clean to sight and touch. (Pf) Corrective Actions: The ice machine was cleaned/sanitized during the inspection and allowed to dry. No visible black mold like substance.//
