Observed 3-large containers of visibly soiled scampi trays with food residue and debris stored for use on prep table in prep area of main kitchen. C/A: Maintain clean and sanitized; equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: Ice discarded, ice bin and dishes cleaned and sanitized Corrected On-Site.    Observed broken floor tiles and disintegrated grouting holding water and food debris between floor tiles throughout main kitchen primarily under dishmachine, 3-bin sink, and cook line. C/A: Repair floor tiles and grout/coving. All physical facilities shall be maintained in good repair. In food service establishments in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch (1 mm). *PIC advised that floor repairs have been discussed and will be completed upon approval.
