    Observed dusty ceiling tiles around vents in warewash area, as well as heavily dusty wall fan. Also observed debris on floor beneath shelves in walk in freezer. PIC advised to clean. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Repeat Violation.
