Equipment food-contact surfaces and utensils shall be cleaned, if used with time/temperature control for safety food, equipment food-contact surfaces and utensils shall be cleaned at least every 4 hours throughout the day. Observed multiple bowls used to toss cooked chicken wings in assorted sauces not cleaned at least every four hours. As per person in charge bowls were cleaned one day prior to inspection. Person in charge removed bowls and washed, rinsed and sanitized bowls. Corrected On-Site. Repeat Violation.

At the time of inspection, an accumulation of m black mold like substance, and encrusted food beverage residue was observed on the ice dispensing shoot of Coca Cola carbonated beverage dispenser and the beverage dispensing nozzles. Person in Charge was advised to, wash , rinse and sanitize the food contact surfaces of teh carbonated beverage dispenser or to contact the manufacture of said equipment tot service said equipment by 8/11/26. Observed a mixture of dirty and clean sauce bottle stored directly in fruit and vegetable prep sink in kitchen. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Observed grease and old food particles accumulated in and under deep fryers in kitchen. Observed heavy grease and old food particles accumulated on kitchen floor around and below deep fryers and grill. Observed walls behind deep fryers and grill soiled with accumulated grease residue.Observed a heavy accumulation of grease residue on ventilation hood filters above cooking equipment. Observed dusty ceiling tiles and/or air conditioning vent covers above 3 compartment sink in kitchen. Observed food residue on walls behind beverage dispensers in Customer Self service area. Observed an accumulation of old food particles, dust and debri on floor of walk in cooler and dust on fan covers of walk in cooler. PIC was advised to clean said areas by 8/11/2026. Repeat Violation.     In all establishments with dining facilities on the premises and permitted since July 31, 1995, patrons' toilet facilities shall be provided. Access to patrons' toilet facilities shall not be through food service, preparation, storage, or warewashing areas. At the time of inspection, toilet facility was observed " Out of Order" and not accessible to customers and one customer was observed eating and drinking in establishment. Person in Charge was advised to have toilet facility repaired by 8/11/2026 in order to allow customers to dine in the facility. New Violation. Correct By: 08/11/2026
