    Observed multiple items cold holding above 41F in reach in cooler and walk in cooler in main kitchen (see asterisk). CA: When cold holding, all TCS food items shall be maintained at 41F or below. EHS recommended that PIC discard items that were over 41F. PIC refused to discard all items. Repeat Violation.

    Observed facility using a screwdriver as closure for the walk-in freezer in main kitchen. C/A Good Repair and Proper Adjustment. 1. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections (1) and (2) of this Rule. 2. Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Facility has 30 days to correct the violation and shall email EHS with confirmation that latch is fixed and no longer requires a screwdriver to stay closed. Repeat Violation. Correct By: 09/17/2026
