   FRP walls in poor repair in kitchen area. Exposed electrical outlet missing cover and missing seamless FRP cover. Ensure remodel application is submitted to this department by 11/6/26. All physical facilities shall be maintained in good repair and shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. (C)
