   One of the two employees on site during the inspection did not have a signed employee reporting agreement. Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: The person in charge had the employee read/sign the employee reporting agreement.

Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. (Pf) Corrective Actions: The person in charge provided hand soap at the sinks during the inspection.
