   Employee health reporting

   Facility unable to provide in a verifiable manner that one out of the two food employees are aware of their responsibility to report the symptoms and/or illnesses. // Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge information about their health and activities as they relate to diseases that are transmissible through food. (Pf) Corrective Actions: The food employee reviewed and signed the employee health agreement form during the inspection with the person in charge (PIC) and general manager.//
